Health Partners’ types of care givers

The caregivers at Health Partners have a variety of certifications and specialties to give your loved one the most qualified care. Below are the types of highly trained caregivers Health Partners works with.


Certified Nurse's Aides (or Assistants) and Home Health Aides

  • All certified nurse’s aides and home health aides have successfully completed a training course, passed both a written and practical exam and placed on a state registry
  • CNAs and HHAs both assist patients with daily activities including personal care, ambulation, nutritional, medications, toileting, health monitoring (i.e. blood pressure) and sometimes light housekeeping. The certified assistant must also be skilled in actual procedures and also make competent observations of a patient's condition for reporting to professionals.
  • The CNAs and HHAs work under the supervision of certified nurses and other health care professionals.

Licensed Practical Nurse (LPN) / Licensed Vocational Nurses (LVN)

  • Licensed Practical Nurses sometimes known LVNs or practical nurses must pass a specific state curriculum requirements and a standardized national exam after completion of their college program to receive their nursing license.
  • They must work under the supervision of a physician or Registered Nurse (RN).They are qualified to perform certain skilled nursing procedures.

Registered Nurses (RN)

  • Our skilled and experience Registered Nurses (RNs) have extensive education and must pass rigorous state curriculum requirements and a standardized national exam after completion of their college program to obtain their nursing license.
  • They are also competent to supervise other members of the health team including LPNs/ LVNs, CNAs, and / or HHAs.

Physical Therapists (PT) / Occupational Therapists (OT) / Speech Therapists (ST)

  • Therapists include: physical therapists, speech therapists and occupational therapists assist in the rehab of those with physical injuries or disease.
  • Their main priority helping patients who are disabled by physical injuries or disease regain maximum function with activities of daily living.

Social Workers (SW) / Medical Social Workers (MSW)

  • Our social workers assist in the evaluation of social, emotional and environmental factors affecting the ill and disabled.
  • They may also provide family or individual counseling for those in need or a crisis situation.
